3. Community endorsement - 20%
4. Availability of health personnel - 10%
5. Faculty evauluation - 5%

Criteria for promotion of nominees for BSN who have been employed (health/health-related work settings):

1. Latest performance evaluation (PES or equivalent in private institutions) - 50%
2. Endorsement from sending agency or sending LGU - 25%
3. Academic standing - 25%

Note: CHW graduates for promotion to BSN should have passed the Midwifery Licensure Examinations and the BSN pre-qualifying examinations conducted by the BSN Department.
